The world of military aviation and special operations often requires its participants to operate continuously for dozens of hours, when concentration and precision are a matter of life and death. In such situations, fatigue is the greatest enemy, and traditional solutions such as caffeine or amphetamines have been found to be problematic over the years due to side effects that include a rapid pulse, irritability, and a sharp decrease in judgment once the effect of the substance wears off.
This is where modafinil comes into play, a drug that was originally developed in France in the 1970s and received medical approval for the treatment of serious sleep disorders such as narcolepsy, characterized by sudden drowsiness during the day. Over time, leading armies in the world, including the American and British Air Forces, discovered that the drug allows pilots to remain alert and focused even after 40 hours without sleep, without harming their motor skills.
The mechanism by which modafinil works is fundamentally different from that of ADHD drugs such as Ritalin or Concerta. While the traditional attention drugs directly and sharply increase the levels of dopamine and noradrenaline in the synapses in the brain, modafinil works in a more subtle and selective way. It affects areas of the brain responsible for regulating states of wakefulness and sleep, especially the hypothalamus. One of its main mechanisms includes raising the levels of the substance orexin, a neuropeptide that plays a critical role in maintaining alertness, as well as an effect on the neurotransmitter histamine.
Unlike caffeine, which blocks the adenosine receptors in the brain to temporarily prevent the feeling of fatigue, modafinil actually produces a state of “pushing” the brain into active alertness. The pharmacokinetics of the drug show that it is well absorbed in the digestive system and reaches the peak concentration in the blood within two to four hours of taking it. Its half-life is relatively long and stands at about 12 to 15 hours, which explains why one dose can keep a person alert for a whole day.
In Israel, the drug is registered under the trade name Provigil, and is defined as a prescription drug intended for specific sleep disorders, including sleep apnea or sleep disorders resulting from shift work. Although it is not officially registered as a drug for the treatment of attention deficit hyperactivity disorder, many doctors note that it has a positive effect on concentration and short-term memory, which makes it popular for “off-label” use among students and professionals in demanding fields.
When a person takes modafinil, they don’t feel the “high” or euphoria associated with strong stimulants, but simply feel alert, as if they’ve had a good night’s sleep, even if they haven’t closed an eye for a long time. This ability to separate physiological arousal from cognitive function is the main reason for its selection for pilots. Studies have shown that pilots who took the drug were able to perform complex maneuvers in simulators with almost the same level of accuracy as when they were resting, while the control group who received a placebo showed a steep decline in performance.
The accepted doses usually range between 100 and 200 milligrams per day, with special military operations sometimes giving a higher dose spread throughout the day. However, the use of the drug is not without risks and side effects. Although it is considered relatively safe and has a much lower addiction potential than amphetamines, it can cause headaches, nausea, dizziness and anxiety.

Meanwhile, one of the significant dangers is the tendency of users to ignore the body’s biological need for sleep. The brain does remain alert, but the body still needs rest for tissue repair and detoxification. When the level of the drug drops in the blood, the user may experience a sharp drop in alertness, which is sometimes referred to as a “crash”, although with modafinil this phenomenon is less pronounced than with other substances. In very rare cases, the drug may cause severe and life-threatening skin reactions, which requires medical supervision during the first intake.
Its use demonstrates the fine line between curative medicine and upgrading medicine, one that tries to stretch the limits of human ability beyond the natural biological limitations. Ultimately, the ability to stay awake for 24 hours straight is a huge tactical advantage, but the physiological price the body pays for stealing sleep hours from the brain is a subject that is still being studied in depth in sleep labs around the world.
